Job Summary
Sherwood Park-Strathcona County Primary Care Network (SPPCN) is seeking a Specialty Referral Coordinator to manage and coordinate specialty referrals from SPPCN physicians, helping patients access specialist care efficiently and effectively. This role supports the referral process by ensuring referral information is complete and accurate, liaising with physician and specialist offices, maintaining confidential medical records, and entering referral and status information in the appropriate Electronic Medical Records (EMR). The successful candidate will demonstrate strong attention to detail, excellent communication and organizational skills, and a commitment to patient-centered, confidential, and professional service.

Responsibilities
As a dedicated member of the Specialty Referral team you will be responsible for but are not limited to the following:
- Advocate for the patient to assist the PCN Physicians in ensuring patients navigate through the health care system as efficiently as possible when accessing specialists.
- Develop effective linkages with Specialist offices and various community health resources to effectively manage patients requiring treatment and referral.
- Ensure referral forms are comprehensive as per the guidelines/requirements of the individual specialist and specialty clinic, this may include:
- Discuss patient status with physicians and/or physician office staff
- Gather information and relay appointment details to patients via telephone
- Retrieve required lab and diagnostic results from netCARE and facilities administering diagnostics
- Enter referrals (i.e. “indexing”) and status information in the appropriate Electronic Medical Records (EMR), including ensuring accurate information is entered
- Maintain confidential medical files and records
- Represent the PCN physicians and the PCN in a professional manner at all times
- Excellent communication skills required, will act as liaison for internal and external stakeholder
- Responsible for ensuring that all statistical data is entered into the EMR for all programs and assist in generating statistical reporting as required
- This role will be responsible to ensure accuracy of the data entry supporting the PCN’s Health Outcome Indicators
- Attention to detail is paramount

- Must be a team player and be able to provide effective communication both verbally and in writing
- Ability to manage multiple tasks
- Strong organizational skills
- Considerable knowledge of health care resources and models of patient care with emphasis on interdisciplinary team approaches
- Ability to deal compassionately and effectively with difficult behaviours sometimes exhibited by patients in distress
- All other tasks required for the smooth and efficient running of the office

- Ability to respect other views/opinions
- Ability to have difficult conversations and be comfortable asking questions.
- Exceptional understanding of and appreciation for confidentiality and privacy
- Ability to identify sensitive information
- Office Administration certificate from a recognized educational institution and a minimum of 5 years of health related experience
- Medical terminology
- Working knowledge of EMR and NetCARE
- Experience dealing with confidential information
- Current Criminal Record Check and Vulnerable Sector Check satisfactory to the organization.
- Valid Alberta driver’s license.
- Access to reliable transportation, as occasional travel may be required.
